Abstract
Obesity is considered a major risk factor for diabetes mellitus type 2; it exacerbates insulin resistance and promotes an inflammatory environment favorable to oxidative stress. The purpose of this study is to determine the normal relationship between dipeptidyl peptidase-4 and heme oxygenase-1 in obese individuals with and without Type 2 diabetes mellitus. 150 blood samples were collected from individuals aged 30 to 65 years included in the study. The individuals were divided into three groups according to their body mass index (BMI). Heme oxygenase-1, dipeptidyl peptidase-4, and insulin levels were measured using the ELISA method, lipid profile, and fasting blood glucose. Insulin resistance was also calculated. An increase in the level of heme oxygenase-1 in serum was found in diabetes mellitus with obesity [(18.0± 0.40) ng/mL, mean ± SE] compared to obesity [(10.41± 0.74) ng/mL], while the concentration of dipeptidyl peptidase-4 was found to be lower in the group of diabetes mellitus with obesity [(7.71± 0.28) ng/mL] and also in obesity [(6.48± 0.27) ng/mL. From these findings, we concluded that a significant positive correlation was observed between heme oxygenase-1 and dipeptidyl peptidase-4 in the obesity and obesity-related diabetes mellitus groups. However, this statistical association does not necessarily indicate a direct biochemical relationship. Heme oxygenase-1 was significantly elevated, whereas dipeptidyl peptidase-4 was significantly reduced in patients with obesity-related diabetes mellitus. ROC analysis indicated that heme oxygenase-1 better distinguished obesity from obesity-related diabetes mellitus, whereas dipeptidyl peptidase-4 showed excellent diagnostic accuracy in differentiating the diseased groups from the control group.
